GLP-1 Side Effects: What to Expect and How to Manage Them
Most GLP-1 side effects are mild, temporary, and manageable. Knowing what to expect — and what to do about it — makes a significant difference in your experience.
Why Do GLP-1 Medications Cause Side Effects?
GLP-1 receptors are found throughout the body — in the gut, brain, pancreas, and heart. When you introduce a GLP-1 receptor agonist, your body needs time to adjust to its effects on digestion, appetite signaling, and metabolism. Most side effects are a direct result of the medication doing its job: slowing gastric emptying and reducing appetite.

Common Side Effects
Nausea
The most frequently reported side effect, particularly in the first 4–8 weeks and after each dose increase. Nausea is typically mild to moderate and tends to improve significantly as your body adjusts.
How to manage it:
- Eat smaller, more frequent meals rather than large ones
- Avoid high-fat, greasy, or spicy foods — they slow digestion further
- Eat slowly and stop when you feel full, even if you have not finished your plate
- Stay upright for 30–60 minutes after eating
- Ginger tea, ginger chews, or over-the-counter antinausea medications (with provider guidance) can help
Vomiting and Diarrhea
Less common than nausea, but can occur especially with dose increases. Usually short-lived. Stay hydrated — electrolyte drinks can help if you are losing fluids.
Constipation
Because GLP-1 medications slow gastric emptying, constipation is common and may persist throughout treatment. Increase fiber intake, stay well-hydrated, and talk to your provider if it becomes uncomfortable — stool softeners are safe to use.
Fatigue
Some people feel tired, especially in the first few weeks. This often relates to eating significantly less and the body adjusting to lower caloric intake. It typically resolves within a month.
Reduced Appetite and Early Satiety
This is the intended effect — but it can catch people off guard. Some patients find it difficult to eat enough protein or calories, particularly early on. Prioritize protein-rich foods and nutrient-dense meals to avoid muscle loss.
Less Common but Notable Side Effects
- Acid reflux / heartburn: Slowed gastric emptying can increase reflux. Avoid lying down after eating and reduce caffeine and alcohol intake.
- Headache: Often related to reduced food intake or mild dehydration. Drink plenty of water.
- Hair thinning: Can occur after rapid weight loss (a phenomenon called telogen effluvium). Maintaining adequate protein intake significantly reduces this risk.
- Injection site reactions: Redness, bruising, or minor swelling at the injection site. Rotate sites and let the medication reach room temperature before injecting.
When to Contact Your Provider

- Severe or persistent vomiting that prevents you from keeping fluids down
- Significant abdominal pain, especially if it radiates to your back
- Signs of an allergic reaction (rash, difficulty breathing, swelling of the face)
- Changes in vision or signs of low blood sugar if you also take diabetes medication
- Any side effects that feel unusual or are affecting your quality of life
Do Side Effects Go Away?
For the vast majority of patients, yes. GI side effects typically peak in the first 4–12 weeks and diminish significantly as your dose stabilizes. In clinical trials, discontinuation due to side effects was relatively low — most patients were able to stay on treatment with support and dose adjustments.
