CalyHealth is not a replacement for emergency medical services. If you are experiencing a medical emergency, seek in-person care immediately or call 911.
1. What Is Telehealth?
Telehealth is the delivery of healthcare services through electronic communications technology — including video, phone, and secure messaging — to facilitate diagnosis, consultation, treatment, education, and care management. By using CalyHealth, you consent to receiving care through telehealth.
You understand that your provider will not be physically present and that all care is delivered remotely. CalyHealth services are not a substitute for in-person care when in-person care is medically necessary.
2. Technology & Privacy Risks
Telehealth involves the transmission of medical information over electronic networks. You acknowledge the following potential risks:
- Technical interruptions, delays, or failures that may affect service quality
- Unauthorized access by third parties despite security measures
- Data processing errors during transmission or storage
- Limitations in the provider's ability to assess your condition without a physical exam
We use industry-standard security protocols to protect your information in compliance with HIPAA. In the event of a technical failure, your provider may contact you by phone to continue your care.
3. Artificial Intelligence Disclosure
Certain aspects of your care delivery may involve artificial intelligence (AI) tools. AI may be used to assist with scheduling, documentation, or administrative functions. AI-generated content is reviewed by licensed clinicians before influencing your care. Sessions may be recorded for quality assurance and provider training purposes. All recordings and AI-processed data are handled in accordance with our Privacy Policy.
4. No Guarantee of Results
CalyHealth does not guarantee any specific treatment outcome. The effectiveness of any treatment depends on individual health factors, adherence to the treatment plan, and other variables outside our control. Your condition may not improve, and in some cases may worsen, even with appropriate treatment.
Your provider may determine at any time that telehealth is not appropriate for your condition and recommend in-person care.
5. Compounded Medications
Some medications prescribed through CalyHealth may be compounded — meaning they are prepared by a licensed compounding pharmacy rather than manufactured by a drug company. You acknowledge:
- Compounded medications are not FDA-approved, though they are prepared in FDA-regulated facilities
- Compounded medications are prepared for individual patients based on a valid prescription
- Risks may include side effects, allergic reactions, and variability in potency or purity compared to commercially manufactured drugs
- You should inform your provider of all current medications, allergies, and health conditions before starting any compounded medication
6. Weight Management Programs
If you are enrolled in a CalyHealth weight management program, you additionally acknowledge:
- GLP-1 medications (such as semaglutide or tirzepatide) have been associated with thyroid C-cell tumors in animal studies; human relevance is unknown
- These medications are contraindicated in individuals with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
- You must notify your provider immediately if you experience persistent hoarseness, difficulty swallowing, a lump in the neck, or shortness of breath
- Weight loss results vary by individual. Typical results require adherence to prescribed medication, dietary guidance, and physical activity
- GLP-1 medications may affect hormonal contraceptives — discuss birth control options with your provider
7. Peptide Therapy
If you receive peptide therapy through CalyHealth (including but not limited to sermorelin, NAD+, or MIC B12 injections), you acknowledge:
- Peptide therapies are typically compounded and not FDA-approved for specific indications
- Results vary and are not guaranteed
- You will follow all instructions provided for self-administration and report any adverse reactions to your provider promptly
- Peptide therapies should not be used without a valid prescription from a licensed clinician
8. Men's Health Treatments
If you receive treatment for erectile dysfunction (ED) or other men's health conditions, you acknowledge:
- PDE5 inhibitors (e.g., sildenafil, tadalafil) are contraindicated with nitrate medications and may cause serious cardiovascular events if combined
- You must disclose all current medications, heart conditions, and relevant medical history before receiving a prescription
- Seek immediate medical attention if you experience an erection lasting longer than 4 hours, sudden vision or hearing loss, or chest pain
9. Shipping Authorization
By using CalyHealth, you authorize medications prescribed to you to be shipped to your address on file. All shipments comply with applicable state and federal laws and require pharmacy approval. You acknowledge:
- Medication is considered dispensed when signed out for shipping, not upon delivery
- CalyHealth is not liable for delays or errors in the shipping process once the medication has been released to the carrier
- You are responsible for ensuring your shipping address is accurate and that someone is available to receive temperature-sensitive medications
10. Communication Authorization
By creating an account and using CalyHealth services, you consent to receive communications from us via phone, text message, and email regarding your care, appointments, and account. Some communications may be generated in part by automated systems. You may opt out of non-essential communications at any time by contacting support@calyhealth.com.
11. Your Rights
You have the right to:
- Withhold or withdraw consent for telehealth services at any time without affecting your right to future care
- Request an in-person evaluation if appropriate
- Access your medical records in accordance with applicable law
- Have your health information kept confidential in accordance with HIPAA
- Receive a copy of this consent
- Ask questions about any aspect of your care
12. Governing Terms
This Medical Consent is incorporated into and governed by CalyHealth's Terms & Conditions and Privacy Policy. By continuing to use CalyHealth services, you confirm that you have read, understood, and agree to this Medical Consent.
